Glavobolje, zbunjenost i delirij koje su neki od oboljelih od covida-19 iskusili mogli bi biti rezultat izravnog prodora koronavirusa u mozak, prema rezultatima istraživanja objavljenima u srijedu.

Istraživanje je preliminarno no pruža nekoliko skupina dokaza u prilog do sada uvelike nepotvrđenoj teoriji.

Prema istraživanju koje je vodio imunolog Akiko Iwasaki sa sveučilišta Yale virus je sposoban umnožavati se unutar mozga i njegova prisutnost oduzima kisik obližnjim moždanim stanicama, no razmjeri pojave još nisu jasni.

Voditelj odjela za neurologiju University of California u San Franciscu Andrew Josephson pohvalio je tehnike koje su korištene u istraživanju i kaže da je "iznimno važno utvrditi postoji li ili ne izravno virusno djelovanje u mozgu".

No, dodaje da želi biti oprezan s obzirom na to da istraživanje nije recenzirano, odnosno zaključke nisu preispitali drugi stručnjaci iz toga područja.

Stručnjaci smatraju da nije posve neočekivano da virus SARS-CoV-2, koji izaziva covid-19, može prijeći iz krvi u moždano tkivo i svladati tu fizičku prepreku čiji je zadatak zaustaviti prodiranje stranih tvari u mozak.

To uspijeva primjerice zika virusu što je dovelo do značajnih oštećenja mozga fetusa.

Liječnici su do sada vjerovali da su neurološki učinci koji su uočeni kod polovice oboljelih od covida-19 rezultat pretjeranog odgovora imunog sustava poznatog kao citokinska oluja koja izaziva i upalu moždanog tkiva, a ne izravnog prodora virusa.

Iwasaki i njegov tim odlučili su pristupiti toj zagonetki na tri načina. Zarazili su mini-mozgove uzgojene u laboratorijskim uvjetima, zarazili su miševe i analizirali su moždano tkivo osoba umrlih od covida-19.

Proučavanjem laboratorijski uzgojenog moždanog tkiva tim je otkrio da je virus SARS-CoV-2 sposoban zaraziti stanice živčanog sustava i potom iskoristiti same neurone kako bi se replicirao.

Zaražene stanice izazvale su odumiranje okolnih stanica tako što su im oduzele kisik potreban za život.

Jedan od glavnih argumenata protiv teorije o sposobnosti virusa da izravno prodre u mozak bio je da mozgu nedostaje dovoljna razina proteina ACE2 na koju se koronavirus veže i koje ima u obilju u drugim organima poput pluća.

No tim je otkrio da u laboratorijski uzgojenom moždanom tkivu ima dovoljno bjelančevine ACE2 da se olakša prodor virusa i da je taj protein postojao i u moždanom tkivu umrlih pacijenata.

Lumbalnom punkcijom hospitaliziranih bolesnika oboljelih od covida-19, koji su imali simptome delirija i kojima se iz kralješničkog kanala uzima iglom uzorak cerebrospinalne tekućine, otkriveno je da postoje neutralizirajuća antitijela protiv virusa u toj tekućini što je bio dodatni dokaz koji je podržao njihovu teoriju.

Tim je genetski izmijenio dvije skupine miševa tako da je jedna imala receptore za ACE2 u plućima, a druga samo u mozgu.

Miševi čija su pluća bila zaražena imali su neke znakove oštećenja pluća, a miševi čiji je mozak bio zaražen gubili su brzo na težini i ugibali što ukazuje na potencijalno veću smrtnost nakon ulaska virusa u mozak.

Na kraju ispitali su mozak troje pacijenata koji su umrli od teškog oblika covida-19 praćenog komplikacijama, te su otkrili dokaze prisutnosti virusa kod svih u različitom stupnju.

U zaraženim područjima nije bilo znakova da su prožeta stanicama imunog sustava poput T-stanica koje pohrle na mjesta na kojima se pojave drugi virusi poput zika virusa ili herpes virusa kako bi ih uništili.

To sugerira da možda pretjerani odgovor imunog sustava poznat kao citokinska oluja, koji je odgovoran za veliki dio oštećenja pluća kod oboljelih od covida-19, možda nije glavni uzrok neuroloških simptoma kod tih bolesnika.

Jedna od hipoteza znanstvenika bila je da nos može predstavljati ulazni put virusa do mozga no autori istraživanja navode da se to mora potvrditi istraživanjima.

Dodali su kako je potrebno provesti više obdukcija kako bi se utvrdili razmjeri moždanih oštećenja kod umrlih od covida-19.

Glavobolje, zbunjenost i delirij koje su neki od oboljelih od covida-19 iskusili mogli bi biti rezultat izravnog prodora koronavirusa u mozak, prema rezultatima istraživanja objavljenima u srijedu.

Istraživanje je preliminarno no pruža nekoliko skupina dokaza u prilog do sada uvelike nepotvrđenoj teoriji.

Prema istraživanju koje je vodio imunolog Akiko Iwasaki sa sveučilišta Yale virus je sposoban umnožavati se unutar mozga i njegova prisutnost oduzima kisik obližnjim moždanim stanicama, no razmjeri pojave još nisu jasni.

Voditelj odjela za neurologiju University of California u San Franciscu Andrew Josephson pohvalio je tehnike koje su korištene u istraživanju i kaže da je "iznimno važno utvrditi postoji li ili ne izravno virusno djelovanje u mozgu".

No, dodaje da želi biti oprezan s obzirom na to da istraživanje nije recenzirano, odnosno zaključke nisu preispitali drugi stručnjaci iz toga područja.

Stručnjaci smatraju da nije posve neočekivano da virus SARS-CoV-2, koji izaziva covid-19, može prijeći iz krvi u moždano tkivo i svladati tu fizičku prepreku čiji je zadatak zaustaviti prodiranje stranih tvari u mozak.

To uspijeva primjerice zika virusu što je dovelo do značajnih oštećenja mozga fetusa.

Liječnici su do sada vjerovali da su neurološki učinci koji su uočeni kod polovice oboljelih od covida-19 rezultat pretjeranog odgovora imunog sustava poznatog kao citokinska oluja koja izaziva i upalu moždanog tkiva, a ne izravnog prodora virusa.

Iwasaki i njegov tim odlučili su pristupiti toj zagonetki na tri načina. Zarazili su mini-mozgove uzgojene u laboratorijskim uvjetima, zarazili su miševe i analizirali su moždano tkivo osoba umrlih od covida-19.

Proučavanjem laboratorijski uzgojenog moždanog tkiva tim je otkrio da je virus SARS-CoV-2 sposoban zaraziti stanice živčanog sustava i potom iskoristiti same neurone kako bi se replicirao.

Zaražene stanice izazvale su odumiranje okolnih stanica tako što su im oduzele kisik potreban za život.

Jedan od glavnih argumenata protiv teorije o sposobnosti virusa da izravno prodre u mozak bio je da mozgu nedostaje dovoljna razina proteina ACE2 na koju se koronavirus veže i koje ima u obilju u drugim organima poput pluća.

No tim je otkrio da u laboratorijski uzgojenom moždanom tkivu ima dovoljno bjelančevine ACE2 da se olakša prodor virusa i da je taj protein postojao i u moždanom tkivu umrlih pacijenata.

Lumbalnom punkcijom hospitaliziranih bolesnika oboljelih od covida-19, koji su imali simptome delirija i kojima se iz kralješničkog kanala uzima iglom uzorak cerebrospinalne tekućine, otkriveno je da postoje neutralizirajuća antitijela protiv virusa u toj tekućini što je bio dodatni dokaz koji je podržao njihovu teoriju.

Tim je genetski izmijenio dvije skupine miševa tako da je jedna imala receptore za ACE2 u plućima, a druga samo u mozgu.

Miševi čija su pluća bila zaražena imali su neke znakove oštećenja pluća, a miševi čiji je mozak bio zaražen gubili su brzo na težini i ugibali što ukazuje na potencijalno veću smrtnost nakon ulaska virusa u mozak.

Na kraju ispitali su mozak troje pacijenata koji su umrli od teškog oblika covida-19 praćenog komplikacijama, te su otkrili dokaze prisutnosti virusa kod svih u različitom stupnju.

U zaraženim područjima nije bilo znakova da su prožeta stanicama imunog sustava poput T-stanica koje pohrle na mjesta na kojima se pojave drugi virusi poput zika virusa ili herpes virusa kako bi ih uništili.

To sugerira da možda pretjerani odgovor imunog sustava poznat kao citokinska oluja, koji je odgovoran za veliki dio oštećenja pluća kod oboljelih od covida-19, možda nije glavni uzrok neuroloških simptoma kod tih bolesnika.

Jedna od hipoteza znanstvenika bila je da nos može predstavljati ulazni put virusa do mozga no autori istraživanja navode da se to mora potvrditi istraživanjima.

Dodali su kako je potrebno provesti više obdukcija kako bi se utvrdili razmjeri moždanih oštećenja kod umrlih od covida-19.

Glavobolje, zbunjenost i delirij koje su neki od oboljelih od covida-19 iskusili mogli bi biti rezultat izravnog prodora koronavirusa u mozak, prema rezultatima istraživanja objavljenima u srijedu.

Istraživanje je preliminarno no pruža nekoliko skupina dokaza u prilog do sada uvelike nepotvrđenoj teoriji.

Prema istraživanju koje je vodio imunolog Akiko Iwasaki sa sveučilišta Yale virus je sposoban umnožavati se unutar mozga i njegova prisutnost oduzima kisik obližnjim moždanim stanicama, no razmjeri pojave još nisu jasni.

Voditelj odjela za neurologiju University of California u San Franciscu Andrew Josephson pohvalio je tehnike koje su korištene u istraživanju i kaže da je "iznimno važno utvrditi postoji li ili ne izravno virusno djelovanje u mozgu".
