Treći val pandemije pokazuje zube i u Hrvatskoj: udio zaraženih među testiranima skočio je na 30 posto, a prema službenoj objavi HZJZ-a stopa smrtnosti od koronavirusa trenutno iznosi 2,2 posto. To znači da će u narednim tjednima na svakih 45 zaraženih u Hrvatskoj jedna osoba umrijeti.

U tim okolnostima cijepljenje stanovništva nameće se kao jedini izlaz iz koronavirusne klopke. No Hrvatska se, kao i brojne druge zemlje, nalazi u problemu jer oxfordsko cjepivo AstraZeneca, kojeg je najviše naručila, stiže na kapaljku.

K tome, zadnjih dana razvila se opsežna kritika ovog cjepiva, poradi nekih njegovih nuspojava - radi se o stvaranju krvnih ugrušaka, odnosno cerebralnoj trombozi venskih sinusa, što najviše pogađa žene, osobito mlađe - što je za posljedicu imalo strah građana i pad njihova interesa za cijepljenje AstraZenecom.

Neke države čak su privremeno prekinule korištenje tog cjepiva, iako je učestalost ove pojave iznimno niska: od 34 milijuna ljudi u EU i Britaniji koji su primili ovo cjepivo, Europska agencija za lijekove (EMA) do 4. travnja registrirala je 169 slučajeva sinusne tromboze i 53 slučaja tromboze utrobnih vena (ukupno 222 slučaja),

što pokazuje koliko su rijetki – sedam slučajeva na milijun cijepljenih (0,0007%), ili jedna tromboza na preko 153.000 ljudi.

Uzimajući sve u obzir, Europska agencija za lijekove (EMA) potvrdila je povezanost cjepiva AstraZenece i stvaranja krvnih ugrušaka, ali je naglasila da je korist i dalje veća od štete.

Hoćemo li zabraniti kontraceptive?

„Nakon istrage više desetaka slučajeva nastanka krvnih ugrušaka nakon cijepljenja AstraZenecom u EU, sigurnosni odbor EMA-e je zaključio da su koristi AstraZenece sveukupno veće od rizika koji donose nuspojave”, rekla je ravnateljica EMA-e Emer Cooke, dodavši da su krvni ugrušci „vrlo rijetka nuspojava cjepiva AstraZenece”.

Iako i EMA navodi da ta nuspojava više pogađa žene, stručnjaci ističu da se krvni ugrušci kod žena puno češće stvaraju ako koriste oralnu kontracepciju – na milijun žena koje uzimaju pilule protiv začeća ima 1100 takvih slučajeva (0,1%), ili jedna tromboza na 909 žena.

Podsjetimo, kod cijepljenja AstraZenecom jedna se tromboza događa na svakih 153.000 cijepljenih. Odnosno 168 puta rjeđe nego kod uzimanja anti-baby pilula. Hoćemo li zabraniti kontraceptive?

Pa ipak, u javnosti se stvara atmosfera da je uzimanje AstraZeneca cjepiva skopčano s velikim zdravstvenim rizicima (njemački odbor za cijepljenje preporučuje da ga se ne daje mlađima od 60, a britanski odbor razmatra spuštanje granice na 30 godina), čime se sugerira da je AstraZeneca cjepivo drugog reda, iako stručnjaci to demantiraju:

– Ove nuspojave nisu razlog da ljudi ne koriste AstraZenecu, jer ta incidencija zaista je mala. Više će ljudi stradati od krvarenja u stomaku zbog uzimanja aspirina, nego od cijepljenja AstraZenecom – kaže nam riječki imunolog dr. Zlatko Trobonjača, te nastavlja:

– Sličnih nuspojava, možda nešto rjeđe, ima i kod drugih cjepiva, pa doista ne znam zašto je ovolika povika baš na AstraZenecu. I u uputama za cjepivo Moderna piše da ga ljudi koji imaju problema sa zgrušavanjem krvi ne bi trebali uzimati – ističe naš sugovornik.

Da sam šef Pfizera...

Među mogućim razlozima svojevrsne hajke na AstraZenecu neki navode i činjenicu da je to cjepivo višestruko jeftinije od ostalih, pa su ga mnoge države naručile u velikim količinama, što ruši profit ostalim farmaceutskim gigantima koji prodaju svoja cjepiva. S cijenom od 1,78 eura po dozi, u odnosu na Pfizerovih 12 dolara ili Moderninih 18 dolara, AstraZeneca je rješenje za sve zemlje tanjeg džepa.

– Čak je duplo jeftinija od ruskog Sputnjika – dodaje Trobonjača, tumačeći da je AstraZeneca jeftina zato što su autori tog cjepiva, znanstvenici sa Sveučilišta Oxford, inzistirali kod proizvođača da cijena, radi veće dostupnosti cjepiva, mora biti svedena na troškove proizvodnje i transporta.

Našeg sugovornika pitamo je li moguće da drugi farmaceutski divovi koriste svoje lobističke moći kako bi u javnosti ocrnili jeftinijeg konkurenta.

– To ne mogu znati. Ali da sam predsjednik uprave Pfizera, zaista bih želio da cjepivo mog konkurenta, koje je približno jednako efikasno, a pet puta jeftinije, nekako doživi krah – kaže riječki imunolog, te rezimira:

– Sva cjepiva daju nam gotovo stopostonu zaštitu od teških oblika bolesti i smrti, a to je najvažnije.

Kako bilo, s obzirom na ozbiljnost situacije, zaključak mora biti matematički. Od korone u Hrvatskoj sada umire svaki 45. zaraženi čovjek, a trombozu od AstraZenece dobije jedan čovjek na 153.000 cijepljenih. Šansa da zaraženi umrete od korone točno je 3400 puta veća nego da od AstraZenece dobijete trombozu.

Treći val pandemije pokazuje zube i u Hrvatskoj: udio zaraženih među testiranima skočio je na 30 posto, a prema službenoj objavi HZJZ-a stopa smrtnosti od koronavirusa trenutno iznosi 2,2 posto. To znači da će u narednim tjednima na svakih 45 zaraženih u Hrvatskoj jedna osoba umrijeti.

U tim okolnostima cijepljenje stanovništva nameće se kao jedini izlaz iz koronavirusne klopke. No Hrvatska se, kao i brojne druge zemlje, nalazi u problemu jer oxfordsko cjepivo AstraZeneca, kojeg je najviše naručila, stiže na kapaljku.

K tome, zadnjih dana razvila se opsežna kritika ovog cjepiva, poradi nekih njegovih nuspojava - radi se o stvaranju krvnih ugrušaka, odnosno cerebralnoj trombozi venskih sinusa, što najviše pogađa žene, osobito mlađe - što je za posljedicu imalo strah građana i pad njihova interesa za cijepljenje AstraZenecom.

Neke države čak su privremeno prekinule korištenje tog cjepiva, iako je učestalost ove pojave iznimno niska: od 34 milijuna ljudi u EU i Britaniji koji su primili ovo cjepivo, Europska agencija za lijekove (EMA) do 4. travnja registrirala je 169 slučajeva sinusne tromboze i 53 slučaja tromboze utrobnih vena (ukupno 222 slučaja),

što pokazuje koliko su rijetki – sedam slučajeva na milijun cijepljenih (0,0007%), ili jedna tromboza na preko 153.000 ljudi.

Uzimajući sve u obzir, Europska agencija za lijekove (EMA) potvrdila je povezanost cjepiva AstraZenece i stvaranja krvnih ugrušaka, ali je naglasila da je korist i dalje veća od štete.

Hoćemo li zabraniti kontraceptive?

„Nakon istrage više desetaka slučajeva nastanka krvnih ugrušaka nakon cijepljenja AstraZenecom u EU, sigurnosni odbor EMA-e je zaključio da su koristi AstraZenece sveukupno veće od rizika koji donose nuspojave”, rekla je ravnateljica EMA-e Emer Cooke, dodavši da su krvni ugrušci „vrlo rijetka nuspojava cjepiva AstraZenece”.

Iako i EMA navodi da ta nuspojava više pogađa žene, stručnjaci ističu da se krvni ugrušci kod žena puno češće stvaraju ako koriste oralnu kontracepciju – na milijun žena koje uzimaju pilule protiv začeća ima 1100 takvih slučajeva (0,1%), ili jedna tromboza na 909 žena.

Podsjetimo, kod cijepljenja AstraZenecom jedna se tromboza događa na svakih 153.000 cijepljenih. Odnosno 168 puta rjeđe nego kod uzimanja anti-baby pilula. Hoćemo li zabraniti kontraceptive?

Pa ipak, u javnosti se stvara atmosfera da je uzimanje AstraZeneca cjepiva skopčano s velikim zdravstvenim rizicima (njemački odbor za cijepljenje preporučuje da ga se ne daje mlađima od 60, a britanski odbor razmatra spuštanje granice na 30 godina), čime se sugerira da je AstraZeneca cjepivo drugog reda, iako stručnjaci to demantiraju:

– Ove nuspojave nisu razlog da ljudi ne koriste AstraZenecu, jer ta incidencija zaista je mala. Više će ljudi stradati od krvarenja u stomaku zbog uzimanja aspirina, nego od cijepljenja AstraZenecom – kaže nam riječki imunolog dr. Zlatko Trobonjača, te nastavlja:

– Sličnih nuspojava, možda nešto rjeđe, ima i kod drugih cjepiva, pa doista ne znam zašto je ovolika povika baš na AstraZenecu. I u uputama za cjepivo Moderna piše da ga ljudi koji imaju problema sa zgrušavanjem krvi ne bi trebali uzimati – ističe naš sugovornik.

Da sam šef Pfizera...

Među mogućim razlozima svojevrsne hajke na AstraZenecu neki navode i činjenicu da je to cjepivo višestruko jeftinije od ostalih, pa su ga mnoge države naručile u velikim količinama, što ruši profit ostalim farmaceutskim gigantima koji prodaju svoja cjepiva. S cijenom od 1,78 eura po dozi, u odnosu na Pfizerovih 12 dolara ili Moderninih 18 dolara, AstraZeneca je rješenje za sve zemlje tanjeg džepa.

– Čak je duplo jeftinija od ruskog Sputnjika – dodaje Trobonjača, tumačeći da je AstraZeneca jeftina zato što su autori tog cjepiva, znanstvenici sa Sveučilišta Oxford, inzistirali kod proizvođača da cijena, radi veće dostupnosti cjepiva, mora biti svedena na troškove proizvodnje i transporta.

Našeg sugovornika pitamo je li moguće da drugi farmaceutski divovi koriste svoje lobističke moći kako bi u javnosti ocrnili jeftinijeg konkurenta.

– To ne mogu znati. Ali da sam predsjednik uprave Pfizera, zaista bih želio da cjepivo mog konkurenta, koje je približno jednako efikasno, a pet puta jeftinije, nekako doživi krah – kaže riječki imunolog, te rezimira:

– Sva cjepiva daju nam gotovo stopostonu zaštitu od teških oblika bolesti i smrti, a to je najvažnije.

Kako bilo, s obzirom na ozbiljnost situacije, zaključak mora biti matematički. Od korone u Hrvatskoj sada umire svaki 45. zaraženi čovjek, a trombozu od AstraZenece dobije jedan čovjek na 153.000 cijepljenih. Šansa da zaraženi umrete od korone točno je 3400 puta veća nego da od AstraZenece dobijete trombozu.
