Zasad nemamo odgovor na pitanje je li britanski, mnogo zarazniji soj koronavirusa već prisutan u Hrvatskoj. Znanstvenici okupljeni na projektu "Varijabilnost sojeva koronavirusa SARS-CoV-2 i genetička podloga domaćina kao biomarkeri za otkrivanje čimbenika rizika tijekom pandemije covida-19", pod vodstvom prof. Kristiana Vlahovičeka s Prirodoslovno-matematičkog fakulteta (PMF) u Zagrebu, dosad nisu detektirali tu varijantu kod nas.

Ako se britanska varijanta proširi u EU i Hrvatskoj, to bi moglo poremetiti planove za procjepljivanje jer se ta varijanta brže širi u populaciji. Ako britanska varijanta postane dominantna i kod nas, prema nekim procjenama trebat će procijepiti 90 posto stanovništva.

"Mislim da se ne trebamo bojati nekakve mogućnosti da bi novi soj mogao izbjeći imunosti postignutoj preboljenjem covida-19 ili vakcinacijom.

Tisuće i tisuće virusnih mutanata neprekidno nastaje, možda i potencijalno opasnijih ili kadrih izbjegavati imunosnom sustavu, no velika većina takvih mutanata ne može opstati jer ovi virusi imaju jednu zadatost, a to je građa proteina šiljka", kaže za Jutarnji list imunolog Zlatko Trobonjača s Medicinskog fakulteta u Rijeci.

Što se trenutačno događa s virusom?

- Nova varijanta koronavirusa, poznata kao B.1.1.7 ili VOC 202012/01, prvi je put zabilježena u Kentu i Londonu 20. rujna 2020. godine. Znanstvenici su tada otkrili čak 17 mutacija u odnosu na originalni soj koronavirusa iz Wuhana. Od tih 17 mutacije, osam je na genu koji kodira ključni S (spike) protein pomoću kojeg virus ulazi u stanicu.

Kako se pojavila britanska varijanta koronavirusa?

- Iako Svjetska zdravstvena organizacija (WHO) nema jasan odgovor na to pitanje, znanstvenici spekuliraju da su se višestruke mutacije dogodile kod jednog pacijenta s jako oslabljenim imunološkim sustavom. Taj se pacijent borio s covidom više tjedana, što je koronavirusu omogućilo da evoluira, a zatim je taj soj zarazio drugu osobu.

Zašto nas varijanta B.1.1.7 toliko brine?

- Zbog visoke zaraznosti: znanstvenici procjenjuju da je britanska varijanta koronavirusa 50 do 70 posto zaraznija nego dosadašnje. Varijanta B.1.1.7 sada je dominantna u Velikoj Britaniji, a proširila se u 52 zemlje svijeta, uključujući i susjednu Mađarsku.

Je li britanska varijanta smrtonosnija od dosadašnjih?

- Prema dosad dostupnim podacima, ta varijanta ne uzrokuje ni teže, ni smrtonosnije kliničke slučajeve od dosadašnjih. No, činjenica je da je puno zaraznija, što znači da će i veći broj ljudi biti hospitaliziran, a to povećava pritisak na bolnički sustav.

Što znamo o južnoafričkoj varijanti koronavirusa?

- Južnoafrička varijanta B.1.351 pojavila se u listopadu i ima potencijalno važnije promjene u proteinu S od britanske inačice. Obje su varijante podjednako zarazne, ali južnoafrička sadrži jednu mutaciju koja bi potencijalno mogla umanjiti učinkovitost cjepiva. Dominantna je u Južnoj Africi, ali se proširila u dvadesetak zemalja.

Koje još varijante koronavirusa izazivaju zabrinutost?

- Znanstvenici upozoravaju na varijantu koronavirusa koja se pojavila u Nigeriji te dovela do naglog rasta broja zaraženih u toj najmnogoljudnijoj afričkoj zemlji te brazilsku varijantu detektiranu u prosincu 2020. u Manausu, glavnom gradu države Amazonije koji je bio jako pogođen prvim pandemijskom valom.

Hoće li cjepiva i dalje biti učinkovita?

- Trenutačna cjepiva dizajnirana su prema ranijim inačicama, ali znanstvenici su uvjereni da bi trebala biti učinkovita i protiv novih varijanti koje se lakše šire. U tijeku su istraživanja koja će to utvrditi. No, čak i u najgorem scenariju, cjepiva bi se mogla redizajnirati, za nekoliko tjedana ili mjeseci, kako bi bila učinkovita i protiv novih varijanti.

Znanstvenici okupljeni na projektu "Varijabilnost sojeva koronavirusa SARS-CoV-2 i genetička podloga domaćina kao biomarkeri za otkrivanje čimbenika rizika tijekom pandemije covida-19", pod vodstvom prof. Kristiana Vlahovičeka s Prirodoslovno-matematičkog fakulteta (PMF) u Zagrebu, dosad nisu detektirali tu varijantu kod nas.

Ako se britanska varijanta proširi u EU i Hrvatskoj, to bi moglo poremetiti planove za procjepljivanje jer se ta varijanta brže širi u populaciji. Ako britanska varijanta postane dominantna i kod nas, prema nekim procjenama trebat će procijepiti 90 posto stanovništva.

"Mislim da se ne trebamo bojati nekakve mogućnosti da bi novi soj mogao izbjeći imunosti postignutoj preboljenjem covida-19 ili vakcinacijom.

Tisuće i tisuće virusnih mutanata neprekidno nastaje, možda i potencijalno opasnijih ili kadrih izbjegavati imunosnom sustavu, no velika većina takvih mutanata ne može opstati jer ovi virusi imaju jednu zadatost, a to je građa proteina šiljka", kaže za Jutarnji list imunolog Zlatko Trobonjača s Medicinskog fakulteta u Rijeci.

Što se trenutačno događa s virusom?

- Nova varijanta koronavirusa, poznata kao B.1.1.7 ili VOC 202012/01, prvi je put zabilježena u Kentu i Londonu 20. rujna 2020. godine. Znanstvenici su tada otkrili čak 17 mutacija u odnosu na originalni soj koronavirusa iz Wuhana. Od tih 17 mutacije, osam je na genu koji kodira ključni S (spike) protein pomoću kojeg virus ulazi u stanicu.

Kako se pojavila britanska varijanta koronavirusa?

- Iako Svjetska zdravstvena organizacija (WHO) nema jasan odgovor na to pitanje, znanstvenici spekuliraju da su se višestruke mutacije dogodile kod jednog pacijenta s jako oslabljenim imunološkim sustavom. Taj se pacijent borio s covidom više tjedana, što je koronavirusu omogućilo da evoluira, a zatim je taj soj zarazio drugu osobu.

Zašto nas varijanta B.1.1.7 toliko brine?

- Zbog visoke zaraznosti: znanstvenici procjenjuju da je britanska varijanta koronavirusa 50 do 70 posto zaraznija nego dosadašnje. Varijanta B.1.1.7 sada je dominantna u Velikoj Britaniji, a proširila se u 52 zemlje svijeta, uključujući i susjednu Mađarsku.

Je li britanska varijanta smrtonosnija od dosadašnjih?

- Prema dosad dostupnim podacima, ta varijanta ne uzrokuje ni teže, ni smrtonosnije kliničke slučajeve od dosadašnjih. No, činjenica je da je puno zaraznija, što znači da će i veći broj ljudi biti hospitaliziran, a to povećava pritisak na bolnički sustav.

Što znamo o južnoafričkoj varijanti koronavirusa?

- Južnoafrička varijanta B.1.351 pojavila se u listopadu i ima potencijalno važnije promjene u proteinu S od britanske inačice. Obje su varijante podjednako zarazne, ali južnoafrička sadrži jednu mutaciju koja bi potencijalno mogla umanjiti učinkovitost cjepiva. Dominantna je u Južnoj Africi, ali se proširila u dvadesetak zemalja.

Koje još varijante koronavirusa izazivaju zabrinutost?

- Znanstvenici upozoravaju na varijantu koronavirusa koja se pojavila u Nigeriji te dovela do naglog rasta broja zaraženih u toj najmnogoljudnijoj afričkoj zemlji te brazilsku varijantu detektiranu u prosincu 2020. u Manausu, glavnom gradu države Amazonije koji je bio jako pogođen prvim pandemijskom valom.

Hoće li cjepiva i dalje biti učinkovita?

- Trenutačna cjepiva dizajnirana su prema ranijim inačicama, ali znanstvenici su uvjereni da bi trebala biti učinkovita i protiv novih varijanti koje se lakše šire. U tijeku su istraživanja koja će to utvrditi. No, čak i u najgorem scenariju, cjepiva bi se mogla redizajnirati, za nekoliko tjedana ili mjeseci, kako bi bila učinkovita i protiv novih varijanti.
