Jedini razlog zbog čega su izv. prof. dr. sc. Vili Beroš, ministar zdravstva, i njegovi suradnici "izmislili" odluku da obiteljski liječnici trebaju brzim antigenskim testom (BAT), u svojim ordinacijama, testirati sumnjive na COVID-19 i da oni koji su pozitivni nakon brzog antigenskog testa mogu dobiti COVID potvrdu, koja vrijedi samo u RH i nigdje drugo, zapravo je novac, odnosno golema ušteda koju će država imati na taj način.

Ta odluka nije epidemiološka, nije u cilju zaustavljanja pandemije i čisto je politička - kazuje dr. Vikica Krolo, liječnica obiteljske medicine, zamjenica predsjednika Hrvatske liječničke komore (HLK) i bivša "šefica" KOHOM-a.

Kako kaže naša sugovornica, druge logike nema.

Samo na otvorenome

- Pa već više od 22 mjeseca, od kada traje ova pandemija, odnosno od kada se ljudi testiraju, uče nas da mi obiteljski liječnici ne smijemo testirati ljude u našim ordinacijama, odnosno u zatvorenim prostorima, jer da na taj način ugrožavamo sve naše ostale pacijente koji dolaze kod nas na preglede. O tome je nedavno govorila i dr. Željka Karin, ravnateljica Nastavnog zavoda za javno zdravstvo Splitsko-dalmatinske županije (NZJZ SDŽ), koja je rekla da se testirati može samo na otvorenom prostoru, odnosno kroz "prozorčić" kontejnera, kako i oni rade na NZJZ-u SDŽ.

I onda odjednom odluka po kojoj testiranja možemo i mi raditi u zatvorenim prostorima, odnosno u našim ordinacijama. Jedino što nam preostaje je ljude testirati izvan ordinacija, da onako lijepo stoje vani u redu i po buri, kiši, jugu... ili "oformiti" ordinaciju na otvorenome - govori dr. Krolo.

U Hrvatskoj se dnevno radi između 16 i 20 tisuća PCR testova

Kako je već istaknula, u pozadini je novac.

- Samo prošli tjedan u Hrvatskoj se dnevno radilo između 16 i 20 tisuća PCR testova. Ako znamo da svaki PCR test košta 380 kuna, jasno je da je cijena tog testiranja samo prošli tjedan bila između 30 i 40 milijuna kuna, ovisno o broju testova. Odnosno, ako se dnevno testiralo 16 tisuća ljudi, to je samo jedan dan koštalo više od šest milijuna kuna, a ako ih se dnevno testiralo 20 tisuća, tada je to dnevno koštalo 7,6 milijuna kuna. A sve to plaća država.

Istovremeno, cijena BAT testova je 20 kuna i sada zamislite da se dnevno BAT-om testira onih 20 tisuća ljudi. Tada to državu dnevno košta 400 tisuća kuna ili na tjednoj razini (pet dana testiranja) dva milijuna kuna. Uštede su ogromne - kaže dr. Krolo.

Zanimalo nas je kako su na ministrovu odluku reagirali obiteljski liječnici diljem RH.

- Većina je reagirala s negodovanjem i neodobravanjem. Mi imamo grupu u kojoj izmjenjujemo svoje stavove i komentiramo sve ono što je vezano uz medicinu. Svi smatraju kako je to izvan svih epidemioloških standarda, odnosno protivno preporukama epidemiologa.

'Izdvojeno' vrijeme

Skoro dvije godine nam "tupe" kako moramo raditi na način da nam u ordinaciju ne dolaze zaraženi ili potencijalno zaraženi ljudi. I tako se mi i ponašamo. Naši "sumnjivi" sami rade BAT testove kod kuće ili idu na PCR test, upravo zbog toga da nam ne bi dolazili u ordinacije. A sada odjednom mogu dolaziti u ordinacije. Znači li to da one koji su donijeli ovakvu odluku nije briga za onkološke bolesnike ili one s kroničnim teškim oboljenjima koji dolaze u naše ambulante i koji se lako od "sumnjivih" mogu zaraziti - pita se dr. Krolo.

Prema preporukama obiteljski liječnici bi trebali ova BAT testiranja obavljati u određeno, posebno za to izdvojeno vrijeme.

- Ma koje je to vrijeme, ono koje mi nemamo? Mi ujutro našim pacijentima vadimo krv, primamo njihove uzorke mokraće i stolice, a nakon toga primamo svakih 15 minuta u ordinaciju pacijente koji su naručeni na pregled.

Krajem radnog vremena, tamo oko 13 sati, počinjemo odgovarati na mailove, SMS-ove ili WhatsApp poruke naših pacijenata, što nismo uspjeli u radnom vremenu. Pa pitam ja ministra, kada da odvojim to "posebno" vrijeme. Možda preko marende, koju nemam? - pita se dr. Krolo.

Još jedan razlog zbog čega nije dobro raditi BAT je činjenica da je on orijentacijski test.

- PCR test je puno bolji test. Neka osoba koja je u ponedjeljak napravila BAT može nakon dva dana biti pozitivna, a da to i ne zna. Zbog toga je puno bolje tri-četiri dana od prvih simptoma napraviti PCR test, on daje pravu sliku i stanje stvari. Ministar Beroš se pita "a tko će raditi BAT testove ako to neće obiteljski liječnici" i kaže da se "na taj način želi zaštititi zdravstveni sustav". Pa što to znači? Da mi obiteljski liječnici i naše medicinske sestre nismo zdravstveni sustav?! Dr. Krolo rješenje problema vidi u otvaranju većeg broja novih punktova za testiranje na otvorenom.

Jedini razlog zbog čega su izv. prof. dr. sc. Vili Beroš, ministar zdravstva, i njegovi suradnici "izmislili" odluku da obiteljski liječnici trebaju brzim antigenskim testom (BAT), u svojim ordinacijama, testirati sumnjive na COVID-19 i da oni koji su pozitivni nakon brzog antigenskog testa mogu dobiti COVID potvrdu, koja vrijedi samo u RH i nigdje drugo, zapravo je novac, odnosno golema ušteda koju će država imati na taj način.

Ta odluka nije epidemiološka, nije u cilju zaustavljanja pandemije i čisto je politička - kazuje dr. Vikica Krolo, liječnica obiteljske medicine, zamjenica predsjednika Hrvatske liječničke komore (HLK) i bivša "šefica" KOHOM-a.

Kako kaže naša sugovornica, druge logike nema.

Samo na otvorenome

- Pa već više od 22 mjeseca, od kada traje ova pandemija, odnosno od kada se ljudi testiraju, uče nas da mi obiteljski liječnici ne smijemo testirati ljude u našim ordinacijama, odnosno u zatvorenim prostorima, jer da na taj način ugrožavamo sve naše ostale pacijente koji dolaze kod nas na preglede. O tome je nedavno govorila i dr. Željka Karin, ravnateljica Nastavnog zavoda za javno zdravstvo Splitsko-dalmatinske županije (NZJZ SDŽ), koja je rekla da se testirati može samo na otvorenom prostoru, odnosno kroz "prozorčić" kontejnera, kako i oni rade na NZJZ-u SDŽ.

I onda odjednom odluka po kojoj testiranja možemo i mi raditi u zatvorenim prostorima, odnosno u našim ordinacijama. Jedino što nam preostaje je ljude testirati izvan ordinacija, da onako lijepo stoje vani u redu i po buri, kiši, jugu... ili "oformiti" ordinaciju na otvorenome - govori dr. Krolo.

Kako je već istaknula, u pozadini je novac.

- Samo prošli tjedan u Hrvatskoj se dnevno radilo između 16 i 20 tisuća PCR testova. Ako znamo da svaki PCR test košta 380 kuna, jasno je da je cijena tog testiranja samo prošli tjedan bila između 30 i 40 milijuna kuna, ovisno o broju testova. Odnosno, ako se dnevno testiralo 16 tisuća ljudi, to je samo jedan dan koštalo više od šest milijuna kuna, a ako ih se dnevno testiralo 20 tisuća, tada je to dnevno koštalo 7,6 milijuna kuna. A sve to plaća država.

Istovremeno, cijena BAT testova je 20 kuna i sada zamislite da se dnevno BAT-om testira onih 20 tisuća ljudi. Tada to državu dnevno košta 400 tisuća kuna ili na tjednoj razini (pet dana testiranja) dva milijuna kuna. Uštede su ogromne - kaže dr. Krolo.

Zanimalo nas je kako su na ministrovu odluku reagirali obiteljski liječnici diljem RH.

- Većina je reagirala s negodovanjem i neodobravanjem. Mi imamo grupu u kojoj izmjenjujemo svoje stavove i komentiramo sve ono što je vezano uz medicinu. Svi smatraju kako je to izvan svih epidemioloških standarda, odnosno protivno preporukama epidemiologa.

'Izdvojeno' vrijeme

Skoro dvije godine nam "tupe" kako moramo raditi na način da nam u ordinaciju ne dolaze zaraženi ili potencijalno zaraženi ljudi. I tako se mi i ponašamo. Naši "sumnjivi" sami rade BAT testove kod kuće ili idu na PCR test, upravo zbog toga da nam ne bi dolazili u ordinacije. A sada odjednom mogu dolaziti u ordinacije. Znači li to da one koji su donijeli ovakvu odluku nije briga za onkološke bolesnike ili one s kroničnim teškim oboljenjima koji dolaze u naše ambulante i koji se lako od "sumnjivih" mogu zaraziti - pita se dr. Krolo.

Prema preporukama obiteljski liječnici bi trebali ova BAT testiranja obavljati u određeno, posebno za to izdvojeno vrijeme.

- Ma koje je to vrijeme, ono koje mi nemamo? Mi ujutro našim pacijentima vadimo krv, primamo njihove uzorke mokraće i stolice, a nakon toga primamo svakih 15 minuta u ordinaciju pacijente koji su naručeni na pregled.

Krajem radnog vremena, tamo oko 13 sati, počinjemo odgovarati na mailove, SMS-ove ili WhatsApp poruke naših pacijenata, što nismo uspjeli u radnom vremenu. Pa pitam ja ministra, kada da odvojim to "posebno" vrijeme. Možda preko marende, koju nemam? - pita se dr. Krolo.

Još jedan razlog zbog čega nije dobro raditi BAT je činjenica da je on orijentacijski test.

- PCR test je puno bolji test. Neka osoba koja je u ponedjeljak napravila BAT može nakon dva dana biti pozitivna, a da to i ne zna. Zbog toga je puno bolje tri-četiri dana od prvih simptoma napraviti PCR test, on daje pravu sliku i stanje stvari. Ministar Beroš se pita "a tko će raditi BAT testove ako to neće obiteljski liječnici" i kaže da se "na taj način želi zaštititi zdravstveni sustav". Pa što to znači? Da mi obiteljski liječnici i naše medicinske sestre nismo zdravstveni sustav?! Dr. Krolo rješenje problema vidi u otvaranju većeg broja novih punktova za testiranje na otvorenom.
